CitizensConnect serves as a convenient tool for Citizens Medical Center patients to quickly, and securely, access their Electronic Health Record (EHR), 24/7. Users can update personal information, view test results and make appointments for mammograms or other tests with just a few clicks on a computer, smart phone or tablet wherever a browser is available. You will need to provide Citizens Medical Center with your email address prior to signing up. The hospital respects your privacy and will not send you spam. You will also need your Medical Record Number, which starts with an M. You may find this number on the CitizensConnect card provided to you upon admission.

Where is my self-enrollment e-mail?
If you have gone thru the self-enrollment process successfully and have not received an e-mail. Check your spam or junk folder in your e-mail client.
Where does my health information in CitizensConnect come from?
All of the information in CitizensConnect comes from your Citizens Medical Center Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.
Can my family access my account?
Yes, you can give family members, such as parents or healthcare proxies, access to your CitizensConnect account. This needs to be done within the Citizens Medical Center Medical Records department, Monday-Friday from 8 a.m. to 5 p.m. and requires consent from you and your family member(s). A photo ID is required to complete the proxy process.
Is my information safe?
Yes. CitizensConnect passwords are encrypted and URLs are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your account. Also, a timeout feature protects your information if you leave the CitizensConnect page open.

What do I do if I lock myself out of my account?
Email CitizensConnect Support at firstname.lastname@example.org and leave your name and phone number. A Citizens Medical Center employee will reset your password. You will receive an email with a new one-time user name and password. Click on the link in the email. Enter your one-time user name and password, answer the security question and click submit.
