
This program is designed to improve the quality of life of individuals suffering from pulmonary disease. The goal of pulmonary rehabilitation is to return these individuals to their maximum level of independence, social interaction and functioning. This is achieved utilizing monitored rehabilitative exercises designed to increase the efficiency of the individual’s pulmonary and muscular systems. The standards have been developed according to national standards established by the American Association of Cardiovascular and Pulmonary Rehabilitation.
How does the Pulmonary Rehabilitation Program work?
There are three phases of pulmonary rehabilitation: Phase I (inpatient-hospital), Phase II (outpatient-rehabilitation), and Phase III (maintenance). The Phase I program is a multidisciplinary effort involving cardiopulmonary, education, dietary and other disciplines as appropriate. The goal of the program is to provide the patient and family with education and prevent serious deconditioning of the patient with chronic pulmonary disease during their hospital admission. Phase II and III pulmonary rehabilitative services are offered in the Functional Restoration area of the Citizens HealthPlex. The goal of the pulmonary rehabilitation program is to return individuals suffering from pulmonary disease to their maximum level of independence, social interaction and function.
The Citizens Medical Center Pulmonary Rehabilitation Program is a monitored program that uses a multi-disciplinary team, consisting of respiratory therapists, physical therapists, nurses and dietitians.
Phase I (inpatient)
— While in the hospital, the Phase I staff will direct your early activity program and provide you with education covering aspects of your disease process and techniques to help manage your condition. Breathing re-training techniques will be taught to help decrease your shortness of breath. Mild to moderate walking, designed to maintain your current endurance level, will be performed while monitoring your heart rate and oxygen level.
Phase II (outpatient)
— Soon after you leave the hospital you can enter the outpatient Phase II program offered at the HealthPlex in the Functional Restoration area. Prior to entering the Phase II program, you will be educated in the aspects of your disease process. During the evaluation process, we will develop a personal plan with you and your family to help manage your condition, directed at strategies to help improve your quality of life. As part of the program, Breathing Re-Training techniques are used to decrease shortness of breath, exercise sessions are offered to increase endurance while monitoring your heart rate and oxygen levels, a lifestyle modification program is designed specifically for you to assist in taking medications, preparing meals and common everyday tasks. Your program will be tailored to your personal needs, ability and interest.On-going evaluations of your progress and continuous communication with your referring physician are provided throughout the program.
The length of the Phase II program is approximately 8 weeks and will be based on medical necessity and your program progress.
Phase III (maintenance)
— The Phase III program is an ongoing option for those that have completed the Phase II program. This program is designed to maintain the functional level and benefits achieved during the Phase II program. The Phase III program, which involves a small monthly membership fee, is an option for individuals who want to continue their exercise program and social interaction.How do you begin a Pulmonary Rehabilitation Program?
Your doctor’s approval is usually all that is necessary to enter the program. After the referral has been made, the Pulmonary Rehabilitation Team can arrange your therapy evaluation. This information, along with your medical history, interests and rehabilitation goals will help us to develop your individualized program. The goals of your program are incorporated into your ongoing care offered by your referring physician.
Pulmonary Rehabilitation (outpatient) is covered by most insurance companies. We would be glad to contact your insurance company to inquire about coverage prior to beginning the program.
